Licensed Practical Nurse (LPN) provides for and assists in meeting the Lake City Care Center MN, TCU (transitional care unit) patients physical, emotional and psycho-social needs while adhering to facility, state and federal guidelines.

Full-time Nursing schedule includes;

• 80 hours in two weeks, Nights
• Includes every other weekend Saturday/Sunday and holiday rotations

Lake City Care Center is owned by the Mayo Clinic Health System with the daily operations led by Ebenezer (MN largest senior living operator) At any given time, as many as 90 people call our LTC and TCU their home.  The care center is connected to a Mayo clinic and hospital, so residents don't have to leave the building for a clinic appointment or emergency care if needed.

• Monitors personal cares provided to assure consistency with each individual plan of care
• Coordinates medications and performs treatments per physician orders and facility policies
• Provides data relevant to the development of the plan of care
• RN – Assesses plan of care, resident goals and revises, modify plan of care as necessary
• Provides reports and documentation related to incidents, change of status and cares provided

Required:

• Current MN RN license by start date
• AHA BLS for Healthcare Providers or Red Cross CPR for Professional Rescuers by start date

Preferred:

• Comfortable using Microsoft windows and electronic charting systems
• Work experience in a patient support role, LPN or RN
• Good written and verbal communication skills to interact with residents, families and physicians or providers
• Ability to work with and have sensitivity to the needs of the geriatric population
