    Aurora on France is a new, state of the art community, attached to Fairview Southdale in the heart of Edina near many medical centers/providers. It is conveniently located off of Hwy. 62 and France Ave. on a bus line. Aurora is a full continuum of care campus including Transitional Care, Enhanced Care and Assisted Living/Memory Care.     The Registered Nurse (RN) provides for and assists in meeting the TCU residents’ physical, emotional and psycho-social needs while adhering to facility, state and federal guidelines. This position is a .8 FTE scheduled 64 hours per/ppd from 6:00 AM to 2:30 PM.  Includes working e/o w/e and holiday.

    Responsibilities/Job Description

    • Managing the TCU patient’s care from pre-admission to discharge and ensuring a smooth transition to and from facility.
    • Provides supervision of direct care staff
    • Patient assessment, development of care plans, and computerized documentation in Point-click Care
    • Coordinates the multidisciplinary care of patients and uses the knowledge to participate as a member of the interdisciplinary team.
    • Contributes information specific to the patient’s response to provide education, medication therapy, pain control, monitoring skin condition, bowel/bladder training, and discharge planning.
    • Provides education for the medical/ surgical conditions of each patient, as well as safe and effective use of medical equipment, proper body mechanics, mobility, transfer, ADLs, and home safety.

    Qualifications

     

    • Minimum Associates Degree, BSN preferred
    • Previous experience in TCU/Med Surge/Acute Care setting preferred but not required
    • Current MN RN/LPN license
    • CPR certified
