Opened in 2018, Cherrywood Pointe of Plymouth is one of our newest senior communities. We are located at the intersection of Old Rockford Road and Highway 55 in the Northwest suburb of Plymouth, MN. We offer 94 units of Independent, Assisted Living as well as Memory Care and Care Suites. The Wellness Coordinator position is responsible for the assessment and documentation of resident care plans and supporting the psychosocial and physical needs of residents.

 

  • This is a full time position, 64 hours every two week pay period.
  • Day and Evening Hours; weeekends as needed.

Responsibilities/Job Description

  • Maximizes incorporation of current assessment and documentation of resident Cognitive Performance Tests, CPT into current care plan. Use the Brief Cognitive Rating Scale to determine resident global deterioration stage. Supports implementation and maintenance of Ebenezer Dimensions program and acts as a Dimensions Champion for the site. 

  • Assists in meeting the psychosocial and physical needs of residents. Develops programs and activities reflective of resident interests and needs. 

  • Trains and monitors work and performance of licensed and unlicensed staff.
  • Support DHS during scheduled times. 

Qualifications

  • Bachelor's Degree; Occupational Therapy; Requred
  • Master's Degree in Occupational Therapy Preferred
  • Certification in Advanced Training of Dementia/Geriatrics; Preferred
  • 1-3 Years of Experience; Required
